Treatment Procedure

  • Review of medical and dental history to ensure treatment is safe and appropriate.
  • Dental examination and X-ray (or 3D CBCT scan if required) to assess tooth position, level of impaction, and proximity to nerves or surrounding structures.
  • Determination of whether the wisdom tooth is fully erupted (simple extraction) or impacted (surgical removal).
  • Patients are advised to have a proper meal before the appointment, especially for surgical cases.
  • Local anaesthesia is administered to numb the area and ensure comfort before the procedure begins.

Simple extraction (erupted wisdom tooth)

  • The dentist gently loosens the tooth using dental instruments and removes it with forceps.
  • The procedure typically takes around 15 minutes per tooth.

Surgical extraction (impacted wisdom tooth)

  • For partially or fully impacted teeth, a small incision is made in the gum.
  • Bone removal and/or tooth sectioning may be performed to allow safe removal.
  • Sutures (stitches) are placed to close the wound.
  • The procedure usually takes 30–60 minutes, depending on complexity.

After removal, bleeding is controlled, sterile gauze is placed, and patients are given post-operative instructions, ice packs, and medication if required.

  • Bite firmly on gauze for about 30 minutes to allow a blood clot to form—this is essential for healing and preventing dry socket.
  • Apply an ice pack intermittently to reduce swelling; cold compresses are recommended for the first 24–48 hours.
  • For the first 24 hours, avoid:
    • Straws
    • Spitting or vigorous rinsing
    • Smoking
    • Hot food or drinks
    • Strenuous activities
  • A soft-food diet (e.g. porridge, soup, yogurt, mashed foods) is recommended for the first few days.
  • For surgical cases, stitches may dissolve naturally or a review appointment is scheduled (around 1-2 weeks) to assess healing.
  • Follow-up checks are done to ensure proper recovery, with prompt management if complications such as infection, prolonged pain, or dry socket occur.

What is wisdom tooth?

Wisdom teeth are the third molars located at the very back of the mouth. They are usually the last teeth to erupt, typically between the ages of 17 and 25.

 

Due to limited space in the jaw, wisdom teeth often do not grow out straight. This is especially common for lower wisdom teeth, which may become impacted (stuck under the gum or bone). When this happens, wisdom teeth can cause pain and lead to various oral health problems.

 

Because wisdom teeth are positioned far back and are often partially covered by gum tissue, they are difficult to clean. Food and plaque can easily become trapped around them, creating a breeding ground for bacteria.

Understanding Impacted Wisdom Teeth

Impacted wisdom teeth occur when there is insufficient space in the jaw for the tooth to erupt normally. These teeth may remain trapped in bone or under gum tissue.

If left untreated, impacted wisdom teeth can:

  • Lead to painful inflammation or infection
  • Form cysts or, rarely, tumours

During surgical wisdom tooth removal, the oral surgeon may:

To promote healing and reduce complications, avoid the following immediately after extraction:

  • Oroantral communication (OAC) (upper wisdom teeth)
    A small opening may form between the extraction site of an upper wisdom tooth and the maxillary sinus due to their close proximity. This can lead to sinus-related symptoms such as fluid passing between the mouth and nose. Most cases are manageable and will be addressed promptly by your dentist or oral surgeon.

When do I need to remove my wisdom tooth?
